Full Time LPN - Medical Coordinator

The Medical Coordinator is primarily responsible for the oversight and management of individuals' physical, medical, and psychiatric needs. The Medical Coordinator assists in coordinating medical, health, and wellness for individuals receiving any I/DD services within a given area. This will also include any specific medical skill or oversight of medical procedures for individuals receiving I/DD services within those areas. This position will be a resource to the team including administrators, directors, managers, site supervisors, staff, etc. The Medical Coordinator will demonstrate and lead all staff in reflecting MERAKEY's mission and core values.
Primary job duties include:
Provide Medical expertise, assistance and guidance to all team members
-Over site of all aspects of Medication Administration of all programs.
-Review and assistance (when needed) of doctor appointments and diagnostic testing.
-Prepare and train on medical topics as assigned or when a need is identified.
-Educate staff, individuals, and/or families on general and specific health care needs.
-Meet with house managers routinely to review medical concerns and needs of individuals.
-Review lab results and ensure follow-up is completed.
-Liaison for pharmacy and pharmacy related issues.
-Assure a safe and hazard free environment for individuals and staff at all times.
-On call support for critical issues.
-Meet with the House Managers in the group home a minimum of once per month.
-Performs medication passes to maintain program integrity
-Maintain current PA licensure as required by state.
-Complete all training requirements, geared to system compliance and professional growth, on an ongoing basis.
-Complete and maintain trainer certification in the Department of Human Services (formally) DPW Medication
-Complete and maintain Certified Investigator status.
-Other trainings as assigned.
-Coordination of Medical Needs
-Contact/liaison for individuals' medical concerns with healthcare providers, families, and team members.
-Participate in coordination of hospital admissions and discharges.
-Coordinate, monitor and/or attend medical appointments to meet program requirements and needs.
-Maintain familiarity with current status and medical needs of all individuals in the program.
-Be familiar with the medical history of all individuals receiving services.
-Identify high risk individuals and ensure specialty care needs are met.
-On-site evaluation of individual as needed.
-Completion of nursing assessments.
-Routinely review documentation to assure all doctor's orders are being implemented.
-Participate in ISP reviews and/or meeting.
-Advocates for proper medical care.
-Clinical and skill development and training for all staff.
-Assist in medication procurement for individuals
Quality Assurance
-Conduct routine nursing assessments.
-Coordinate and documents necessary follow up to incidents as required.
-Routinely audit records to assure all requirements are met.
-Assist in all regulatory and licensing activities.
-Responsible for the review of medical documentation for accuracy and quality.
-Maintain files and databases as assigned.
-Participates as a Medication Administration Trainer which may include the following: teaching the course (medication administration and practicum observer), performing monthly reviews of Medication Administration Records, and signing off on the necessary documents to ensure Medication Trained Staff trainings do not expire.
-Participate in QA activities as assigned. To include medical chart audits, participate in mock drills, and medication audits.
-Data entry and review of medical information as assigned.
-Familiarization and utilization of the electronic record system, assistive technology, etc.
-Assures all program and site compliance to MERAKEY standards and all regulations.
-Complete Certified Investigations as assigned.
-Understands and maintain compliance with all MERAKEY policies and procedures.

Valid Pennsylvania Licensed Practical Nurse or Registered Nurse license with a minimum of one (1) year experience, preferably in medical/surgical or community health nursing.
Current CPR Certification (American Red Cross prefered), Criminal Clearance, Child Abuse Clearance, valid Driver's License.
Experience caring for individuals with IDD and knowledge of DW 6400, 6500, 2380 regulation preferred. Supervisory experience a plus.
Position requires excellent verbal and written communication skills and creativity in problem solving.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
